PCN Staff
Primary Care Network (PCN)
The Primary Care Networks are a key aspect of the NHS Long Term Plan.
- GP practices are part of a network and generally cover between 30,000 to 50,000.
- Our PCN consists of Oak Tree Health Centre, Didcot Health Centre and Woodlands Medical Centre.
- The aim is to build on existing primary care services to enable a more proactive, personalised, coordinated and a more integrated health and social care.
- An example of our PCN working together. The hugely successful Didcot Vaccination Programme. Where the three surgeries coordinated and worked together to complete over 50,000 vaccinations at the local Civic Hall.
Clinical Pharmacists and Pharmacy Technicians
Clinical pharmacists work in primary care as part of a multidisciplinary team in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
They work with and alongside the general practice team, taking responsibility for patients with chronic diseases and undertaking clinical medication reviews to proactively manage people with complex polypharmacy, especially for the elderly, people in care homes and those with multiple comorbidities.

Physiotherapists
Physiotherapists are qualified autonomous clinical practitioners who can assess, diagnose,
treat and manage musculoskeletal (MSK) problems and undifferentiated conditions and – where appropriate – discharge a person without a medical referral. Physiotherapists working in this role can be accessed directly by self-referral or by staff in GP practices who can direct patients to them to establish a rapid and accurate diagnosis and management plan to streamline pathways of care.
Physiotherapists – Vicki and Jide
Physiotherapists providing a first point of contact service means that patients presenting with a musculoskeletal problem for a GP appointment are offered an appointment with a physiotherapist instead. Physiotherapists working in general practice are able to address the needs of a large proportion of the patient population. They have the clinical expertise and autonomy to assess, diagnose and treat patients with a range of conditions, including MSK, neurological and respiratory conditions.
Physician Associates
Physician Associates (PAs) are healthcare professionals with a generalist medical education who work alongside doctors providing medical care as an integral part of the multidisciplinary team.
Physician Associates are dependent practitioners who work under the supervision of a fully trained and experienced doctor. They bring new talent and add to the skill mix within teams, providing a stable, generalist section of the workforce which can help ease the workforce pressures that the NHS currently faces.
