The NHS Healthier Together app provides clear information about what to look out for if your child is unwell and if needed it will allow you to inform us about your child's symptoms rather than having to call to get an appointment. Download the app here

Children Under 18

If your Appointment is about your child, we advise you use the Healthier Together app. This provides expert advice to support you in obtaining appropriate healthcare for your child.

Adult GP Assessment System

If you need help with a non urgent medical problem, we will ask you to complete a quick and easy online form. 

Please include as much information about your condition as possible to help the doctor with assessing your query and getting you to the right person to help you. You can complete the form here

If you don’t have access to a computer, you can still ring the surgery and we will complete the form for you.

All forms will be reviewed by a GP within 24 hours.

Our aim is to improve access and support patients to get the right help from the right professional in the right timeframe. 

 

Nurse Appointments

The following appointments are available to book online via the NHS app or Patient Access

  • Cervical Smear: Must have been invited, please note this is for screening not to investigate health symptoms
  • Blood Tests: Only if requested by a GP or hospital
  • Depo Injections
  • Zoladex/Prostate or other hormone injections
  • Contraception reviews

For all other routine nurse or HCA appointments please use the routine care request option via our online form or call reception in the usual way.

 

Sickness Certificates (Fit Notes)

You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays. For a new fit note please complete an online form to request a call from a GP. If you need to extend or re-new an existing fit note please complete an admin request via the online form.

Home Visits

Home visits are available for patients who are housebound because of illness or disability.

If you believe that you require a home visit you should telephone the surgery between 9:00am and 10:30am.  

A Doctor will phone you back as it may be that your problem can be dealt with by telephone advice or that it would be more appropriate to send a nurse, the Early Visiting Service or to arrange a hospital attendance.

GP Home visits are made after morning surgery usually between 12:00pm to 2:00pm (these times may vary).

It is not possible to guarantee that the doctor of your choice will visit. 

 

Change or Cancel an Appointment

Please give us as much notice as possible so we can offer your appointment to someone else.

Out of Hours

Enhanced Access

Ask at reception for enhanced access information or check the Oxford Health NHS Foundation Trust website

 

Life Threatening 

Call 999 or go to A&E now if:

  • you or someone you know needs immediate help
  • you have seriously harmed yourself - for example, by taking a drug overdose

A mental health emergency should be taken as seriously as a medical emergency.

  • You have an urgent medical issue requiring on the day attention

Non-urgent

Use NHS 111 if:

  • You need help now, but it's not an emergency

There will be someone to provide you with advice and to direct you to a clinician if it is necessary.
