Practice introduction
Oak Tree Health Centre was opened in 1997 to offer a comprehensive range of health care services to the people of the surrounding area, and in particular the residents of the Ladygrove Estate. In Jan 2002 we moved from temporary accommodation to new purpose-built premises.
We aim to provide a happy, friendly, family service, whilst at the same time, aiming to be at the forefront of general practice and provide a first-class service to our patients.
The Health Centre is located in the heart of the Ladygrove area with ready access from all directions. We are close to the main shopping area, convenient for the local schools and other amenities, and with a dispensing chemist near by.
We pride ourselves on a friendly working atmosphere and wish to provide a welcoming environment for all.
Access is available for the disabled throughout our premises: a lift is fitted.Disable parking spaces are available adjacent to the main entrance.

To register at the Oak Tree Health Centre we would prefer if you could pop in and fill in the Family Doctors Services Registration Form (GSM1) along with our own Patient Registration Forms.
Patients registering outside the EU please bring your identification and UK clearance documents with you so we can take a copy.

OAK TREE PATIENT PARTICIPATION GROUP 2011
We have received a very positive response to our request to patients who would be interested in joining Oak Tree's Patient Participation Group (PPG).
The Health and Social Care Bill of 2011 heralds an exciting time in primary health care; a central goal is for General Practices to become the commissioners of local health services, and to ensure that we provide the highest quality input to this process Oak Tree's PPG will form an essential part.
We have now held 2 meetings of our new Patient Participation Group and more meetings are being planned. We will be publishing the activities of this Group in the very near future.

FIT NOTES have replaced SICK NOTES
The FIT NOTE allows doctors to advise patients who have been on sick leave for more than seven days whether they can return to work more quickly. The system allows doctors to advise that an employee may be fit for work or that he or she may be able to return to work with some support from his or her employer. If an employee is too ill to work, the doctor will continue to advise this, as at present.
